A 65 yr old female presented with itching

This is a case of 65yr old female came from nlg homemaker by occupation came with 

Cheif complaints of itching since  5yrs

Hopi 

Patient was apparently asymptomatic 5yrs ago then she developed itching over the left leg below the knee (she noticed hyperpigmented patch ) intially it was small later it covered  most of the anterior surface of the leg 

Since 1month she is experiencing more itching 5yrs ago she went to the hospital where they prescribed medicine but didn't subside 

2-3months ago she developed hyperpigmented patch over the back of the lower abdomen and groin

She always scratch her lesion for relief which cause minimal bleeding

And she put water on her lesion for relief 

There is history of photosensitivity 

Past history

Not a k/c/o diabetes, hypertension

No history of blood transfusion.

Personal history

Appetite -normal 

Diet-mixed 

Sleep -adequate 

Bowel and bladder movements -regularGeneral examination


Pallor:absent

Icterus:absent

Clubbing: absent 

Cyanosis: absent

Lymphadenopathy: absent 

Edema: absent 


Vitals

Temp: afebrile on time of examinatio

BP 120/80mm hg

Heart rate 90 bpm

Resp Rate 14 /min 

Respiratory examination

Inspection: Chest bilaterally symmetrical, all quadrants

moves equally with respiration

Palpation: Trachea central, chest expansion normal

Percussion: Resonance

Ascultation: B/l equal air entry

NVBS

CVS examination

Inspection:Apical impulse visible 

Palpation :Apical impulse felt at 5th intercostal space 1cm medial to midclavicular line  

Ascultation: S1 S2 are heard No murmur 

Abdominal examination:

Soft ,non tender

 Shape of the Abdomen: distended

Umbilicus -inverted

Bowel sounds are heard

No organomegaly 

CNS examination:

HIGHER MENTAL FUNCTIONS- 

Normal

Memory intact

CRANIAL NERVES :Normal

SENSORY EXAMINATION

Normal sensations felt in all dermatomes

MOTOR EXAMINATION

Normal tone in upper and lower limb

Normal power in upper and lower limb

Normal gait

REFLEXES

Normal, brisk reflexes elicited- biceps, triceps, knee and ankle reflexes elicited

CEREBELLAR FUNCTION

Normal function

No meningeal signs were elicited
